To request full article click here. OBJECTIVE: To report a suspected case of subclinical neuroleptic malignant syndrome (NMS) that possibly responded to electroconvulsive therapy. CASE SUMMARY: A 51-year-old white woman with a history of schizoaffective disorder, depressed type, was admitted to our inpatient unit after presenting with all features of NMS except fever. The patient's symptoms were markedly improved after eight courses of electroconvulsive therapy. DISCUSSION: The "spectrum concept" of NMS is increasingly being recognized.
